All partner agencies must:

Complete a brief interest form and sign a partnership agreement outlining expectations
Refer families who have secured stable housing
Participate in mutual outcome sharing to support transparency and impact

As part of this partnership, we will:

Share relevant outcomes and insights from our work with referred families
Request basic follow-up data from you to support evaluation and improvement
Maintain open communication to ensure families receive the best possible support
